(I'm not sure, but I suspect this is affecting those who started NG+ before 1.20/1.21?). There's a bug that only affects some people in that if your current equipment is unequipped, it will suddenly "jump" to a higher level and you find you can't wear it any more. On the left, it still gives the original +30 level, but on the right it gives the new one (and, I think, it may actually craft sometimes at something different again). There's a bug that I think affects everyone in that the crafting screen seems unreliable in telling you what level gear is. However, enemies don't "auto-correct", so if you start the game with a very high level Geralt, say Level 70, you could find yourself facing enemies 20 or more levels above you late in your playthrough. With equipment, I *think* from what I've seen that it will correct itself, and that if you craft something that claims to be (say) level 105, it will actually become level 99. Quest "recommended levels", equipment and enemies can go above Level 100, but Geralt can't. So far, I think these are the issues that have been mentioned: If you started your NG+ game before 1.20/1.21 came out, it stays with whatever was happening, but there seem to be various bugs.
